Integration Pioneer Sites
Providing ambitious and innovative approaches to efficiently deliver integrated care
In 2013, national partners asked local areas (NHS and local government together) to express an interest in becoming Pioneers to act as exemplars, demonstrating the use of ambitious and innovative approaches to efficiently deliver integrated care. More information can be found on the NHS England website.
NHS Kernow used their funding for the training of Living Well facilitators who are supporting front-line practitioner teams to co-design and develop integrated working across health, social care and the voluntary sector in several localities. The Living Well approach is a key element of Cornwall’s Sustainability and Transformation Plan. System Leadership Training has taken place for 16 clinicians across the system.

South Devon and Torbay Clinical Commissioning Group (CCG) has designed and developed a strategic workforce plan for the future that is supporting the development of an employer network across Torbay to deliver the integration agenda.
